CONSENT FORM TO PROCESS PERSONAL INFORMATION IN TERMS OF THE PROTECTION OF INFORMATION ACT, 4 OF 2013 (POPI)
- I, hereby give my consent to Employment Solution Management, as well as any associated employer, Krutham, the NSF, the Department of Higher Education and Training, the Presidency of the Government of the Republic of South Africa, the Auditor-General of South Africa, the South African Revenue Service and the Parliament of the Republic of South Africa (inclusive of any relevant Portfolio Committees) herafter referred to as “The Companies” to collect, process, distribute and release my personal information where The Companies are legally required to do so, and in the event of legitimate business interest.
- I understand my right to privacy and the right to have my personal information processed in accordance with conditions for the lawful processing of personal information.
- I acknowledge that I understand the purposes for which my personal information is required and for which it will be used.
- I hereby declare that I understand that third parties will have access to my personal information and I hereby give consent to The Companies and their group of companies/partners to share my personal information strictly for purposes of reporting to relevant Executive Authorities and Public regulatory bodies, and/or in accordance to Employer POPI, PAIA, or related Policies and Procedures.
- I understand that should I refuse to provide The Companies with the required consent and/or information, The Companies will be unable to assist me regarding registration requirements for a workplace-based learning Programme.
- I understand further that all my personal information which I provide to The Companies will be held and/or stored securely for the purpose for which it was collected.
- I declare that all my personal information supplied to The Companies for the relevant employmet/learning Programme, and related legal and operational reasons are accurate, up to date, not misleading, and complete in all respects.
- I undertake to immediately advise The Companies of any changes to my Personal Information should any of these details change.
- I understand that the facility and classrooms are under 24-hour surveillance for health and safety & quality assurance mentoring purposes.
